Telehealth Informed Consent & Waiver

You are asked to accept this consent before a physician reviews your intake. It explains how asynchronous telehealth works, its benefits and risks, and the responsibilities you accept as a patient.

Nature of telehealth care

Your evaluation is asynchronous: you submit a written medical intake, photos, and (where required) lab results, and a licensed physician reviews them without a live visit. The physician may request additional information, labs, or a synchronous visit before prescribing, and may decline to prescribe.

Benefits

  • Access to a licensed clinician without travel or waiting rooms.
  • Typically same-day or next-business-day clinical review.
  • Ongoing messaging with your care team for dose adjustments and side-effect management.

Risks and limitations

You acknowledge that telehealth has limitations compared with an in-person visit:

  • No physical examination is performed, so some conditions may be missed or misdiagnosed.
  • Care depends entirely on the accuracy and completeness of the information you provide.
  • Technology failures or delays can interrupt communication.
  • Medications carry risks, including allergic reactions, side effects, and interactions with other drugs.
  • Results vary between individuals and no specific outcome is guaranteed.

Your responsibilities

  • Provide accurate, complete, and current health information, including all medications and supplements.
  • Follow the prescribed protocol and dosing instructions, and do not exceed the prescribed dose.
  • Attend or complete any labs or follow-up your physician requires.
  • Report side effects promptly, and seek emergency care for severe symptoms.
  • Do not share, resell, or transfer your medication to anyone.

Assumption of risk and waiver

You voluntarily choose telehealth care and assume the risks described above. To the fullest extent permitted by law, you release Nuvara Health from claims arising out of inaccurate or incomplete information you provide, your failure to follow prescribed instructions, or your use of medication in a manner not prescribed. This waiver does not apply to professional negligence by a treating clinician, and does not limit any right that cannot be waived under applicable law.

Alternatives and right to withdraw

In-person care is always an alternative. You may withhold or withdraw this consent at any time without affecting your right to future care; withdrawing consent ends the telehealth relationship going forward.

Records and privacy

Your telehealth records are maintained as part of your medical record and protected under HIPAA and our Notice of Privacy Practices. Visits are not recorded on video.

By checking the consent box at checkout or in your intake, you confirm that you are 18 or older, are located in a state where our clinicians are licensed, have read and understood this consent and waiver, and have had the opportunity to ask questions.
