HIPAA Notice of Privacy Practices

Our commitment

Nuvara and its affiliated medical groups are required by law to maintain the privacy of your Protected Health Information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

Uses and disclosures without your authorization

We may use or disclose PHI for the following purposes:

• Treatment: sharing your intake and history with the physician, pharmacist, or lab involved in your care.
• Payment: processing your payment and documenting services rendered.
• Health care operations: quality review, clinical audits, staff training, and compliance.
• As required by law: public health reporting, adverse-event reporting, court orders, and health oversight.
• To avert a serious and imminent threat to your health or the safety of others.

Uses that always require your written authorization

• Most uses and disclosures of psychotherapy notes.
• Marketing communications that involve PHI.
• Any sale of PHI — which Nuvara does not do.

Your rights

• Inspect and get an electronic or paper copy of your medical record, usually within 30 days.
• Request corrections to information you believe is incorrect or incomplete.
• Request confidential communications by a specific method or address.
• Request restrictions on certain uses or disclosures.
• Receive an accounting of disclosures made in the prior six years.
• Receive a paper copy of this notice on request.
• Be notified promptly if a breach compromises your unsecured PHI.

Complaints

You may file a complaint with our Privacy Officer at privacy@mynuvara.com, or with the US Department of Health and Human Services, Office for Civil Rights, at 200 Independence Avenue SW, Washington, D.C. 20201, or hhs.gov/ocr/privacy/hipaa/complaints. We will never retaliate against you for filing a complaint.

Changes to this notice

We may change this notice and apply the revised terms to all PHI we maintain. The current version is always posted on this page with its effective date.
